The Recreation Center Pass application form is in PDF format. You must have Adobe Reader installed to use this document.

• To move from field to field, you may use the tab key on your keyboard or click in the field with the mouse.
• To print, click on the button at the bottom of the form.
• This form may be completed online and printed, or printed and completed by hand.
• You may mail or hand-carry the application to the Recreation Center, along with the appropriate payment.
• You may use the form to request a new pass or to renew an existing pass.
• Photos for ID cards will be taken at the Recreation Center.
• Renewals will be updated automatically. No new card will be issued.

Remember, all applications must be accompanied by an appropriate form of payment, or your application will not be processed.
